Evaluation Request

Note:  You should have already submitted your application form and received feedback from our staff prior to filling out and submitting this evaluation request form.

This is a partially refundable fee based on your child's evaluation results and acceptance into our school.  If during the evaluation any concerns are noted and you choose not to move forward with enrollment, a partial refund of $150 will be made within 5 business days.

The fee covers administrative processing and enrollment setup.  Also included in this fee is the cost of conducting your child’s academic and functional skills evaluation. The evaluation helps us determine the appropriate level of support and begin the creation of the individualized learning plan.
